A recording medium capable of data rewrite on a write-once type recording medium so as to improve the usefulness of the write-once type recording medium. The write-once type recording medium includes a main data region having a normal recording/reproduction region (user data region), a rewrite replacement region (OSA), and a replacement management region (ISA). When a data write is requested to an address where data has been recorded, the rewrite data is recorded on the rewrite replacement region (OSA) and replacement management information relating the original address to the address in the write replacement region is recorded in the replacement management region (ISA), thereby realizing data rewrite. That is, a write-once type recording medium can be used substantially as a data rewritable recording medium.
