<P>PROBLEM TO BE SOLVED: To make a medical examination efficient by rationarizing input in an electronic chart. <P>SOLUTION: A medical chart sheet having a mark sheet filling column at least including alternatives regarding a medical examination result of a doctor is provided at a part of the medical chart sheet. Furthermore, a system having a means for optically reading the medical chart sheet and a storage means for storing read information by associating it with patient information is included. Furthermore, a system including a means for rearranging the alternatives in an order of use frequency, a means for preferentially outputting the rearranged alternatives from the one with high use frequency and the one used last are included.
