PROBLEM TO BE SOLVED: To provide a delivery slip, the manufacturing and handling of which are easy and, at the same time, which can be reduced in size as a whole. SOLUTION: This delivery slip comprises a pasting slip 4, on the rear surface of which an adhesive 2 and a releasing paper 3 covering the adhesive 2, an under side slip 6 pseudo-adhered on the front surface of the pasting slip 4 and a top side slip 7, which is superposed on the top side of the under side slip 6 and bound tearingly at one side edge.
