PROBLEM TO BE SOLVED: To provide a metallic form to which a separator can be easily installed and which has separator insertion holes through which ready mixed concrete does not leak out and which is easily bendable while keeping the strength. SOLUTION: Slits for adjusting the installation height of the separator are continuously formed at the lower end of the large diametral reverse egg shaped insertion holes 20 in which the collar of the separator can be inserted to prevent ready mixed concrete from leaking out when shutting and a blind cover having a bending strength withstanding opening/closing to fix the screw of the separator is provided. In this way, the separator can be inserted from the outside of the panel 1 after the form panel 1 has been erected and further, a bending guide 40 with round holes at both the upper and lower ends can be foldably installed at a specified distance.
