PROBLEM TO BE SOLVED: To provide a method for forming the uneven pattern plate surface of a form for molding a concrete block in which a photosensitive resin with a part exposed to ultraviolet rays or sunlight cured and an unexposed part left water-soluble is used, a conventional form can be used, a pattern can be changed easily, and cost can be reduced. SOLUTION: A resin film for adhesion 2 is formed on a plate surface for forming the pattern of the form 1. A frame 3 is mounted on the plate surface, and the photosensitive resin 4 is cast in the frame 3. A negative film 6 expressing the pattern 5 is adhered to the upper surface of the resin 4. The negative film 6 is exposed to ultraviolet rays or sunlight. The film 6 and the frame 3 are removed, the unexposed part 4B of the resin 4 is washed off, and the exposed part 4A is dried.
