PROBLEM TO BE SOLVED: To provide a low-cost trocar for an operation under an endoscope that can guide the endoscope, a surgical instrument, or the like obliquely to the paries and effectively prevent slip-off with simple constitution while maintaining airtightness in a body cavity. SOLUTION: This trocar 1 for the operation under the endoscope is provided with a guide tube 6 mounted in a piercing manner to the paries to guide the endoscope and the surgical instrument into the body; a flange part 5 provided at the guide tube 6 at a prescribed angle to a plane almost orthogonal to the axial direction of the guide tube 6, and fixed to the surface of the paries; and a sealing means 7 provided at the guide tube 6 to airtightly hold a space between the peripheral surface of an inner hole of the guide tube 6 and the endoscope and surgical instrument inserted through the inner hole of the guide tube 6.
