<P>PROBLEM TO BE SOLVED: To provide a questionnaire creation support system, capable of simply creating, answering and accumulating a desired questionnaire. <P>SOLUTION: A server 5, a customer terminal 15 and a respondent terminal 17 are connected to each other through a network 13. The server 5 has a means for transmitting a questionnaire creation screen to the customer terminal 15, a means for creating a questionnaire using data transmitted from the customer terminal 15, a means for transmitting the created questionnaire to the respondent terminal 17, and a means for accumulating the answer to the questionnaire. The customer terminal 15 has a means for displaying a questionnaire creating screen transmitted from the server 5, and a means for transmitting the data input to the questionnaire creating screen to the server 5.
