PROBLEM TO BE SOLVED: To allow a user to find out a timing of the need for maintenance. SOLUTION: A scale corresponding to estimated durable years of a building is provided in a sheet; names of members included in external facing members and buried equipment members, which serve as regions to be maintained, are printed on a sheet in a direction orthogonal to the scale; a figure, which indicates the maintenance timing of each of the members included in the external facing members and the buried equipment members, is printed along the direction of the scale; and a space for entering a maintenance cost is printed.
