PURPOSE: Provided is a name card form for a computer printer which allows a lot of name cards to be printed on one name card form and separated piece by piece easily so that it helps to produce many name cards with reducing production cost. CONSTITUTION: In the name card form for the computer printer which consists of a name card paper(10) which is possibly printed by a computer; a background paper(20) which is possibly attached to the name card paper; a cutting line(30) which allows a few of cut name cards(10a) to be adhered; a polyethylene layer(19) which is applied on the upper side(20-1) of the background paper; an adhesive layer(11) which is applied under the lower side(10-2) of the name card paper; and a peeling agent layer(12) which is applied on the upper side(19-1) of the applied polyethylene layer.
