PROBLEM TO BE SOLVED: To provide a vision examination chart capable of simultaneously and simply performing a plurality of vision examinations items, facilitating carrying, without requiring a large installation type apparatus, nor a space and special environment for examination like a semi-dark room which is an examination apparatus taking light as an index, and examined in a designated posture, and easily examining a patient even if he can't rise. SOLUTION: This examination chart 1 is formed by lattice indexes 3 of lattice stripes in which square lattices are arranged in vertical and horizontal directions, and circular indexes 4 displayed in the respective lattices of the lattice indexes 3. The insides of the circular indexes 4 are colored in hue different from the background. |
