PROBLEM TO BE SOLVED: To provide an observation item adjusted to the condition of a patient in the scene of home-visit nursing care. SOLUTION: This device has a nursing care field and an observation item corresponding to an observation step (obtained by quantizing the difference of observation items corresponding to the state of the patient and discriminating it into multiple steps). The observation steps in the respective nursing fields of the patient are specified from the nursing observation record and the past observation course of the patient by referring to a static nursing field table without depending on the patient. Then, the observation item of the patient at next visiting time is decided by referring to the nursing care field table.
