PROBLEM TO BE SOLVED: To prevent a set operation of character information relating to a false personal identification number not identical with a personal identification added to face image information of a person requesting the issuance and also to provide an IC card or the like of high reliability and a high image quality. SOLUTION: The present equipment has a hopper 11 and a carrying belt device 12 supplying a card member 30, a plurality of image forming units 13 for colors which form a color face image on the card member 30 carried by the carrying belt device 12 on the basis of the face image information D1 relating to the corresponding personal identification number ID, a color image forming unit 18 for monochrome which forms a character image on the card member 30 having the color face image formed, on the basis of the character information D2 relating to the personal identification number ID, and a control unit 90 which controls supply carriage of the card member 30 in the hopper 11 and the carrying belt device 12 and an image forming process in the image forming units 13 and 18 so as to check the identity of the personal identification number ID. |
