PROBLEM TO BE SOLVED: To provide a sheet for an envelope with a self-addressed envelope for replay having excellent folding suitability by a folding machine and easily inserting into the self-addressed envelope of the return sheet. SOLUTION: The sheet 1 for the envelope with the self-addressed envelope for reply comprises an upper sheet 2 and a lower sheet 3 adhered at three peripheral edges, tearing perforations provided at an inside of the adhering part in a top and bottom direction end, a sealing part 7 for the self-addressed envelope for reply formed at the opening end of the sheet 3, a one side opening type envelope 10 with the self- addressed envelope at a boundary between the perforations 8 and 9 for cutting at the adhering end sides of the sheet 2 and the sheet 3, sealing ends 14 and 15 of the self-addressed envelope along the perforations 9 at the opening end side of the envelope 10, an intermediate sheet 22 of the self-addressed sheet having a smaller side than those of the sheets 2 and 3 separately adhered to the sheet 3 in the state in which the sheet 22 is inserted into one end side in the envelope 10, a forward address information display area 3c and a forward address sender information display area 3d provided on pats of front and rear surfaces as the envelope at the front surface side of the sheet 3.
