PROBLEM TO BE SOLVED: To provide a damping device for an ultrasonic surgical treatment of which constituting elements for damping undesired vibration are arranged in an ultrasonic wave guide. SOLUTION: An ultrasonic surgical apparatus of the invention is equipped with internal damping devices such as an ultrasonic wave guide 20, an acoustic insulation element 75, an outer sheath 30, a fitting hub 40, and a division-type sheath 57. The ultrasonic wave guide 20 has a blade end effector 22, a dent part 32, a wave guide pinhole 23, a hollow 21, and a fitting screw 28. The outer sheath 30 has an opening 33, a sheath pinhole 24, a sheath pin slot 29, and a hub engagement part 31. The fitting hub 40 has a trench flat part 46, a sheath slot 44, and a pin slot 42. An O-ring 25 is given such a size as to mechanically insulate the ultrasonic wave guide 20 from the outer sheath 30 in the dent section 32. The O-ring 25 and the acoustic insulation element 75 support the ultrasonic wave guide 20 in the outer sheath 30.
