A supply cartridge and a withdraw cartridge are removably attached to a slide mount carrier. In the supply cartridge, plural slide mounts are stacked. In the slide mount carrier, a discharge mechanism and a multi-feed prevention plate are provided. When an engage craw of the discharge mechanism moves back and forth under the supply cartridge, one of the slide mounts is push out through a mount outlet of the supply cartridge. The multi-feed prevention plate is biased by a spring to cover an upper side of the mount outlet. The multi-feed prevention plate is pressed when the slide mount is discharging. <IMAGE>
