PROBLEM TO BE SOLVED: To simply set newly issued record entry sheets by collecting newly issued record use entry sheets distributed every month at one place and to reduce the number of peel papers to be used. SOLUTION: A large number of pasteboards 6 are connected in series from the free end side of at least one of covers 4 and 5 through hold parts 7 and made developable to be inserted in the gaps between the covers 4 and 5, and the text 3 and newly issued record data entry sheets 8 are together bound to the pasteboards 6 along at least one sides of them. |
