PROBLEM TO BE SOLVED: To provide a form apparatus capable of repeatedly manufacturing a half PC panel having a predetermined strength and dimensions of a thickness, a width and a length as designed by allowing a manufacturing space to be narrow and performing a simplification of a manual work after placing a concrete. SOLUTION: The form apparatus for manufacturing the half PC panel 5 in which a prestress is introduced comprises a bottom form 13 for molding an end face 5c of the board at one side of a lateral direction, a first side form 17 stood at one side of the form 13 in the lateral direction to mold a smooth surface on one side surface 5a of the board, a second side form 22 stood at the other lateral side of the form 13 for molding rugged strips 9a and 9b for shear connectors toward a lateral direction at the other side 6b of the board, a pair of end forms 30 and 30 for molding an end 6c of the board in a longitudinal direction, and a PC steel material 35 for introducing the prestress over the pair of the forms 30 and 30.
