<p>A business form (110) incorporates an improved multi-layer integral card (120) utilizing micro-perforations to form ties (N) which retain the card during machine handling and printing. The method employs a flexible sheet die (132) or engraved die cylinder (133) with microperforations. The die (132) penetrates all layers but the bottom laminate (115) and the microperforations form ties (N) to secure the card (120) until removed for use. The combination and method of forming allows convenient removal of the card by an end user with edges on the removed card (120) being smooth to sight and touch.</p> |
