PROBLEM TO BE SOLVED: To provide a superior sample storage tool provided with the simple diagnostic function for the status of skin and simply specifying a sample of the kind optimum to a customer. SOLUTION: The whole of the sample storage tool 1 is formed into a booklet shape by folding one sheet into two and further folding a double-folded section thus formed into two through a first fold and a second fold in parallel with the first fold and provided with an insertion pocket 3 storing samples 4 and 5 on a surface section 2 of the booklet body, and two choices displaying questions and answers are displayed on a first double-spread face to be appeared by opening the surface section 2 of the booklet sideways, and notches are formed respectively in the vicinity of respective choices, and check columns are provided on respective sections facing the notches on a second double-spread face to be appeared by opening the first double-spread face sideways, and descriptions regarding cosmetics corresponding to respective choices are displayed respectively in the vicinity of respective check columns.
