PROBLEM TO BE SOLVED: To provide a simple and clear investigation record table which solves the problem of difficulties in grasping the contents of reports reaching 300 pieces to 500 pieces and grasping work locations for adapting to emergency as investigation even on one route reaches a few kilometers to tens of kilometers when taking pictures, investigating what type of an accident is at what distance from the start, of what degree, how emergent repair should be performed, etc., and obtaining samples for adaptation through the use of a robot from a manhole for the investigation of a sewer main. SOLUTION: In the record table, a heading route number and document number are recorded in the uppermost part, and record columns for a work name, vendor, construction location, constructor, and construction date are provided below the uppermost part. Columns for at least a joint part, main part, and socket part are provided further below. Record columns for numbers, picture numbers, anomaly contents, and distances and columns for anomaly contents and anomaly locations are sequentially provided in the record part. Investigation contents and ABC ranking are printed by letters separated by different colors.
