PROBLEM TO BE SOLVED: To obtain a delivery slip capable of dealing with both information output of a printer and information description with handwriting by carrying the slip with handwriting information to be copied in the slip planely developed from a plurality of cut forms. SOLUTION: A base material 2 obtained by separably connecting a laminating cut form 5 and the delivery slip 6 respectively having delivery information fill-in parts through a perforation line 7 is separably superposed on the upper surface of a copy sheet 3 having a copying mechanism such as a self-color developing or the like and covering an adhesive part 9 provided on the lower surface with a release sheet 10 through a pseudo-adhesive layer 8. Then, a second laminating slip 11 having a delivery information fill-in part 4 in which information described in the part 4 of the cut form 6 is to be simultaneously copied is separably partitioned and constituted at the corresponding position of the slip 6 of the sheet 3.
