PROBLEM TO BE SOLVED: To use meditation results induced from one theme while putting them in order in a manner to be easily visible. SOLUTION: On a form paper 1, main units 2 and subsidiary units 3 which are arranged in a manner to surround the main units 2 are printed. The main unit 2 comprises a theme-displaying frame 5, and a plurality of meditation result displaying frames 6 which respectively display a plurality of meditation results induced from the theme. The subsidiary unit 3 comprises a new theme displaying frame 15 which displays each meditation result as a new theme, and new meditation result displaying frames 16 which are arranged in a manner to surround the new theme displaying frame 15 and respectively display a plurality of new meditation results induced from the new theme.
