Preparation for repairing cartilage defects or cartilage/bone defects in human or animal joints is described. The bone part of the preparation is formed in such a way that the subchondral bone plate and the cartilage layer of the implanted preparation are supported on regions with different breakdown properties. Independent claims are included for: (A) preparation for repairing cartilage defects or cartilage/bone defects in human or animal joints, comprising (i) a bone part, (ii) a cartilage layer which grows a cartilage layer on the bone part and (iii) a subchondral bone plate (or a facsimile of such a plate) which is situated in the transitional area between the cartilage layer and the bone part. The preparation can be implanted in an opening or boring in the defective region in such a way that the bone component is anchored in healthy bone tissue while the cartilage surface is aligned with a native cartilage tissue surface. The bone part of the preparation is formed in such a way that the subchondral bone plate and the cartilage layer of the implanted preparation are supported on regions with different breakdown properties. (B) repair of cartilage defects or bone/cartilage defects in human or animal joints, using a preferred preparation as described above, comprises: (a) construction of an opening or boring in the defect region, where the opening or boring reaches as far as healthy bone material; and (b) implanting the preparation into the opening or boring. The opening has an external region and an internal region. The cross-section of the external region is bigger than the cross-section of the internal region. The preparation fits the opening in such a way that it can be implanted with pressing and in such a way that the cartilage surface of the implanted preparation aligns with the native cartilage surface. The preferred preparation comprises a head component (which comprises the cartilage layer and the subchondral bone plate) and a foot component (which corresponds to the bone part). The subchondral bone plate is bigger than a parallel cross-section through the foot component, so that the subchondral bone plate of the implanted preparation is supportable by native bone tissue. (C) tool for preparing a preferred preparation as described in (B), comprising an agent for positioning a cylindrical pre-preparation and an agent ofr reducing the cross-section of the pre-preparation in the area of the bone part. (D) tool for preparation of a two-stage opening or boring for implantation of a preferred preparation as described in (B), comprising a blade with a circular cutting edge and two moveable borers with different diameters. |
