PROBLEM TO BE SOLVED: To provide a six-sheet type delivery slip used by a dealer of which a sheet used as an affixing slip and an absence slip is peelable, which enables reuse of a corrugated cardboard box and makes it possible to cope well with the absence of a receiver on the occasion when freight is delivered and of which a receiver's copy slip can be kept in custody or the like separately. SOLUTION: This delivery slip is comprised of six sheets and the forms thereof are put on each other and bound at the ends on one side. The slip is constituted of a client's copy slip 1 of the first sheet, a sales slip 2 of the second sheet, a first form A of the receiver's copy slip, a delivery completion slip or the like of the third sheet, a second form B of the delivery completion slip, the receiver's copy slip or the like of the fourth sheet, the affixing slip and absence slip C of the fifth sheet on the lower surface of which a re-peelable adhesive 11 is so applied as to surround the four sides of the outer periphery thereof and which has a duplicate part 9a for the receiver of a duplicate frame 9 and the absence slip 6 having a duplicate part 9b for a client are made separable into halves practically, and a dealer's copy slip 7 of the sixth sheet on the upper surface of which a peeling layer 12 is provided by coating at a position corresponding to the re-peelable adhesive 11 and wherein the external shape of the duplicate frame 9 is formed to be equal to or smaller than the internal shape of the peeling layer 12.
