SOLUTION: This load delivery form 1 used for load delivery control comprises a load-stuck attached tag 5 having a radio tag storing at least the information showing a unity and having a function transmitting the information to the outside and the bar code information corresponding to the information showing the unity and stuck on a load and a delivery control attached form 4 having the bar code information corresponding to the information showing the unity and a destination column for entering a destination of the load and used for the load delivery control.
