PROBLEM TO BE SOLVED: To provide a delivery slip, in which privacy is protected and also the value of a delivery article can not be judged from the delivery slip during delivery when concealed information is the value of the delivery article. SOLUTION: The delivery slip 1 consists of a sticking form 2 and a concealed slip 5. The sticking form 2 is constituted by entering delivery information 16 on the surface and providing adhesives 4 on three peripheral edges of the rear. The concealed slip 5 is constituted as follows. Both a delivery form 10 entering delivery information 19 on one side and concealed information-entering formes 12, 13, in which receipt information 20 of money including concealed information such as the value of a delivery article and receipt information 21 are entered on the same side, are separably joined. A remover 6 is provided in three peripheral edges of the surface and the surface side thereof is overlapped on the rear side of the sticking form 2 and stuck by the adhesives 4, Also, cutting parts 7 are provided in the three peripheral edges and when these are cut off, the adhesives 4 of the sticking form 2 are exposed and sticking is enabled together with the sticking form 2. On one hand, the delivery form 10 and the concealed information-entering forms 12, 13 are separated from the sticking form 2.
