PROBLEM TO BE SOLVED: To propose a concrete segment form structure for forming concrete products to be securely connected with one another through connecting members. SOLUTION: For the purpose of forming a structure by connecting respective concrete products one another through connecting members, a form 1 is used for forming simultaneously spaces for disposing connecting members one another on a part of concrete products when they are formed, and each form is formed of a bottom member 2 and side frame member 3, and cores 4 are provided on the lower sections of the inner faces of the side frame members through plate members, and washers having tapered surfaces and nuts having tapered surfaces corresponding of the tapered surfaces of the washers are disposed inside the cores, and bolts are screwed from the outside of the side frame members into the nuts, and the cores are brought into close contact with the inner faces of the side frame members and the upper face of the bottom member and fixed thereon by tightening bolts.
