PROBLEM TO BE SOLVED: To preserve a file such as a patient's case record or the like for a long period, to suppress a damage of its bar code even by using the file and to facilitate reading or positioning of the code by printing or adhering the code to a central position or its vicinity of a folding line in a doubled file. SOLUTION: The file 1 such as a patient's case record or the like is formed of a cover 3 and a rear cover 4 obtained by folding a cardboard at a folding portion 2. Fill-in columns (ruled lines) of patient's information or the like are printed on the cover 3 and the cover 4, and a bar code 5 is printed at a central position of the portion 2. This code 5 is obtained by encoding, for example, a patient's case record number or a patient's ID, and an Arabic numeral 6 of the same content as the encoded number is attached to an end of the cover 3. Meanwhile, paper fasteners 7, 8 are respectively attached to a left end of the cover 3 and an upper end of the cover 4, and a prescription or the like is bound with their insides.
