The log book comprises several pages in the form of a journal, diary or question and answer document and is for use in a two-part relationship between an institution, workplace or authority represented by a first person and a number two person. An essential part of the printed text is divided into two identical sections (2,3) divided by a weak line (1), enabling the log book to be divided into two identical parts for each of the two participants in its use. The two sections are provided with two or more holes (4), so that the logbook can be accommodated in a filing system.
