Pernasal preparations characterized by containing a compound of general formula (I) having an Na/H exchange inhibiting activity or a salt thereof exhibit excellent bioabsorbability and an Na/H exchange inhibiting activity superior to those of peroral preparations, thus being useful as preventive and/or therapeutic agents for ischemic heart diseases such as myocardial infarct and arrhythmia: wherein A is an optionally substituted five- or six- membered aromatic heterocycle; B is an optionally substituted five- or six- membered aromatic homo- or hetero-cycle; R1 is hydrogen, hydroxyl, or lower alkyl; and n is 0 or 1.
