Procedure in which the person filling out the form is guided between form fields that require filling out. A clear and unambiguous question is associated with each field to be filled out and displayed with each field are tips, examples and any associated rules or regulations. When the form has been filled out it can be printed off and or forwarded to a databank over a computer network.
