This invention is methods, and devices for forming a conduit (18) to place a target vessel in fluid communication with a source of blood, such as the aor ta or a heart chamber. The conduit (18) may include a first conduit (118) including synthetic vascular graft material, such as PTFE, coupled to a seco nd conduit (110) in the form of a hollow tissue structure, such as an autologou s vessel.
