PROBLEM TO BE SOLVED: To enhance productivity by arranging upper and lower reinforcing bars in advance in a form, and folding connecting frames and the upper and lower main reinforcing bars between both side plates of the form. SOLUTION: In this form A comprising a plurality of connecting frames 2 arranged inside a pair of right and left side plates 1, 1 with both ends of each connecting frame 2 freely rotatably connected to both side plates 1, 1 by appropriate means, upper main reinforcing bars 4 and lower main reinforcing bars 5 to be arranged in respective predetermined positions on the upper and lower sides of each connecting frame 2 are freely rotatably connected to the cross bars of the connecting frames 2 via appropriate support means without obstructing folding of the connecting frames 2. The upper main reinforcing bars 4 and the lower main reinforcing bars 5, together with the connecting frames 2, are folded between the side plates 1.
