PROBLEM TO BE SOLVED: To obtain integrally a return envelope and an initial envelope from one sheet and to easily form a double envelope by inserting an order slip, etc. in the return envelope. SOLUTION: Each of sheet parts 2, 3, 4, 5 is provided with a first sealing function-forming piece 7. The sheet parts 4 and 5 are provided with second sealing function-forming pieces 9. The superposed facing face 10 of the sheet parts 3 and 4 is adhered peelably. The two sides excluding each folded side of the superposed facing face 12 of the sheet parts 4 and 5 are made to adhere strongly. A sealing agent applying part 16 is provided on the superposed facing face 15 of the first seal function-forming piece 7 of the sheet part 2. A peeling agent applying part 18 is provided on the superposed facing face 17 of the sheet part 2 side of the first sealing function-forming piece 7 of the sheet part 3. A seal strengthening agent apply part 20 is formed on the superposed face 19 on the sheet part 4 side of the first seal function-forming piece 7 of the sheet part 5. A sealing means 22 is provided at least on one side of the superposed facing face 21 of the second seal function-forming piece 9 of the seat part 4 and 5.
