PROBLEM TO BE SOLVED: To easily form a high molecular polymer optical waveguide at a low cost. SOLUTION: A polymer base body 11 containing a radical producing agent is prepared as a first clad. A part of the body 11 where an optical waveguide is to be formed is irradiated with UR rays 15. The body 11 after irradiation of UV rays is brought into contact with a first radical polymerizable monomer 19 which becomes a polymer after polymn. having a higher refractive index than that of the body 11 so as to form a polymer part as a core 21 where the optical waveguide is to be formed. A sample after the core 21 is formed is brought into contact with a second radical polymerizable monomer 23 which becomes a polymer after polymn. having a same refractive index as that of the body 11 so as to form a polymer part as a second clad 25 on the core 21.
