PROBLEM TO BE SOLVED: To provide a form rich in designing and functionality as a formwork to integrate with a concrete structure, not requiring to use a heavy machinery, free to manually construct and easy to work in accordance with a work execution site and its work execution method. SOLUTION: As an example, a form 1 forming a wall surface by integrating it with a concrete structure is constituted of a fiber reinforcing board 2 and a reinforcing board 3 made of a steel sheet adhered on one surface of this fiber reinforcing board 2 by adhesive. Additionally, a plural number of the accumulated formworks are integrally connected to each other on the side of their back surfaces by accumulating the forms in this constitution so that the reinforcing boards or designing material parts come to be their front surfaces and using bolts and a plural number of fixtures set in bolt insertion holes provided on the forms. Thereafter, work is executed by integrally fixing a plural number of the accumulated forms by installing adjustable fittings free to revolve which are separately provided on a plural number of the fixtures on separators to fix the forms.
