PROBLEM TO BE SOLVED: To provide a report form for recording a clear print of seal on the form using a pigment type color developing sheet layer. SOLUTION: An applicant column 4 and a seal column 5 of the report form each has a pigment type color developing sheet layer 12, a release sheet layer 15, and a surface form 16. A rear surface of the form 16 is coated with an adhesive 18, releasably adhered to the layer 15, a rear surface of the layer 15 is coated with an adhesive 17, a release sheet 30 is releasably adhered to a position of the seal column and adhered to the layer 12. A rear surface of the layer 12 is coated with an adhesive 19, and adhered to a predetermined column of the form body 1. To use the form, the layer 12 is sealed, and a first print of seal 40 is recorded. Then, the sheet 30 is released, the layer 15 and the layer 12 are adhered, and a second print of seal 41 is recorded by the same seal on the form 16. The columns 4 and 5 are adhered to a ledger or the like by releasing the form 16 from the layer 15.
