PROBLEM TO BE SOLVED: To reduce the labor for filling a commodity information on a gift application form, prevent troubles caused by the erroneous filling of a commodity code and reduce the labor and expense required for the assortment of distribution slips. SOLUTION: A system is provided with a first reading means 16 for reading information filled on a gift application form, a second reading means 17 for reading commodity codes printed on a label stuck on the gift application form a slip data forming means 18 for binding pieces of information read by the first and second reading means 16 and 17 for respective addresses to be delivered rearranging pieces of information for respective addresses based on the commodity codes and forming a distribution slip forming data for respective addresses to be delivered and a printer 19 for printing the distribution slip forming data on slip papers.
