PROBLEM TO BE SOLVED: To accurately guide a treatment tool even without providing an endoscope with a treatment tool insertion channel, by piercing plural insertion holes in the axial direction of a guide member insertably and detachably inserted inside a jacket pipe and turning at least one insertion hole to an endoscope insertion hole. SOLUTION: After forming an insertion route from the body surface skin into the celom by building an inner needle in the jacket pipe 1, the inner needle is pulled out, the guide member 3 is inserted to the jacket pipe 1 and an endoscope 20 or the like is guided into the celom by the guide member 3. The guide member 3 is composed by successively providing a connection part 3b turned to a large diameter on the base end part of a pipe part 3a whose outer diameter is made slightly smaller than the inner diameter of the jacket pipe 1 and the tip part of the guide member 3 is projected from the tip of the jacket pipe 1. Then, the plural insertion holes 5, 6 and 7 are pierced in the axial direction of the guide member 3, the insertion hole 5 is turned to the endoscope insertion hole for inserting the endoscope 20, the insertion hole 6 is turned to a treatment tool insertion hole for inserting forceps 40 and the insertion hole 7 is turned to a gas passage for supplying pneumoperitoneum gas.
