PURPOSE:To totally integrate a model plan by calculating a cost required for a care plan and a care service depending on the attribute and the level of the care service obtained from replies of a questionnaire and generating the model plan in which the cost required for the care service and the care plan are corresponded to each other. CONSTITUTION:A registration number is automatically provided to each reply for each reply person when the reply obtained with respect to questionnaire items is inputted by using an input section and the reply is stored in an auxiliary storage section as succeeding reply data (steps 101, 103). Then it is discriminated that a future address indicates an accurate address based on the stored reply data (step 105) and when not accurate, a current address is set as the future address (step 107). Then an average remaining age is calculated (step 109), and the possibility of entrance to an installation at the future address and the possible resident period are calculated (steps 111, 113). Then the public resident service is retrieved (step 115). |
