PURPOSE:To obtain an antimicrobial agent, containing a novel cephem derivative, capable of exhibiting excellent antimicrobial activity against Gram-negative and Gram-positive bacteria and having high safety. CONSTITUTION:A novel antimicrobial agent consisting of 7beta-[(Z)-2-(5-amino-1,2,4- thiadiazol-3-yl)-2-methoxyiminoacetamido]-3-( 4-carbamoyl-1-quinuclidinio )methyl- 3-caphem-4-carboxylate or a salt thereof. The cephem derivative is obtained by reacting a compound expressed by formula I (X is halogen) with a compound expressed by formula II in a solvent, e.g. methylene chloride, etc., at -10-60 deg.C and, if necessary, removing the protecting group. When this compound is used as an injection, the dose thereof is 100mg-10g a day intravenously or intramuscularly administered in 1-4 divided portions.
