PURPOSE:To enable a correct tracking operation to be conducted without increasing a manufacturing cost. CONSTITUTION:Exciting end parts 31a, 32a of tracking wave guides 31, 32 and the exciting end part 11a of an information recording/reproducing wave guide 11 are formed on the surface of a substrate 6. Furthermore, positions of tracking spots 35, 36 and a recording/reproducing spot 48 are formed while being shifted to the longitudinal direction of the recording grooves by arranging the substrate 6 obliquely so that the surface of the substrate 6 intersects with the longitudinal direction of recording grooves 42, 43 with theta angular degrees. |
