The flexible replica grating (6) comprises a metal layer (3) of a wave-like shape, a synthetic resin layer (4) for supporting the metal layer (3), and a flexible synthetic resin film (5) for supporting the synthetic resin layer (4). The flexible replica grating (6) is preferably used for an optical multiplexer/demultiplexer comprising a substrate (11), a slab waveguide (12), the flexible replica grating attached to the convex end of the slab waveguide (12), and an optical fiber array (17) attached to the other end of the slab waveguide (12).
