PURPOSE: To disclose a medical device to act so as to record physiological data to show a body condition of a patient and phenomenal data to show a function performed by a device. CONSTITUTION: A device 10 contains a microprocessor 20 to prepare a patient report by inserting recorded physiological data and phenomenal data into a template of the patient report stored in a ROM 42. The microprocessor 20 converts the patient report into a series of facsimile compatible codes so as to be transferred to a remote facsimile device through a modem and a telephone interface 72.
