PROBLEM TO BE SOLVED: To provide an apparatus for forming a molding form for a stone board-integrated block wherein in manufacture of the stone board-integrated block, forming and detaching of the form can be efficiently carried out without requiring much labor. SOLUTION: In an apparatus for forming a molding form of a stone board- integrated block for molding a stone board-integrated block wherein the stone board is integrated on a surface of a concrete block by pouring concrete, a stone board-supporting frame 3 for supporting an edge of the stone board 2 from below is arranged to a frame 1. An upper molding frame 4 and a lower molding frame 5 for molding the upper part and the lower part of the block 11 which can move on and back in a lateral direction, are oppositely arranged. A pair of side molding forms 8 and 9 for molding a side part of the block 11 which can move on and back in a vertical direction of the frame 1 are oppositely arranged. A molding frame of a measure shape is formed by allowing the stone board supporting frame 3, the upper molding frame 4, the lower molding frame 5 and the side molding frames 8 and 9 to come in contact with each other.
