PURPOSE:To accurately and quickly acquire all necessary information by providing the recording areas for the index information for individual identification of owners and the image information and enabling each owner to carry these recording areas individually. CONSTITUTION:A recording medium is provided with a user area for users to record and reproduce the information. Thus the individual medical treatment information A, B, C... are recorded together with the directory information a, b, c... corresponding to those medical treatment information respectively. In addition, the information (individual index information) on the face photo of the owner of a relevant optical disk, etc., is also recorded in a specific area of the user area. Thus the information on each individual is recorded to the recording medium and therefore a large quantity of information can be scattered for reduction of the quantity of information to be retrieved. Furthermore the index information is displayed in an image so that the accurate correspondence is secured between the owner of the information recording medium and the information recorded into the medium. Then the information is quickly and accurately retrieved. |
