A medical recording system is disclosed as having a dispenser (11) having an array of compartments; a plurality of pre-printed recording stickers (20) having on one surface an anatomical designation and on the opposing surface an adhesive means; said array of compartments being sized and configured to hold an array of said stickers with at least a portion of the designation visable on a sticker in each compartment so that the stickers can be visually identified while positioned in the compartments, wherein a physician can view the array of stickers in said dispenser during a physical examination and selectively remove those stickers which relate to the physician's progress report to document the findings of the physical examination.
