PURPOSE: To simply and quickly and write by allowing an information surface to have an almost helical and/or concentric format and allowing the information surface to be read out when a card is made rotated around at least one axis. CONSTITUTION: A rectangular information card 10 has a surface relief structure being a mechanical read possible surface at least one the surface of one side of a base plate 12 and a protective layer is overlapped on the information surface 16. The base plate 12 and the protective layer makes the electromagnetic radiation of a selected range which can be used for the optical readout of the information suface 16 transmit. The information surface 16 is arranged by forming the almost concentric and/or helical format and the information surface 16 is allowed to be read out when the card 10 is rotated around the axis. Thus, the reading and the writing are easily and quickly performed.
