PURPOSE:To form (copy) an image directly on a large-sized form or notebook by forming a passage for a recording medium continuously from its supply opening to its discharge part and extending it to a flank crossing the flank of the supply opening at right angles to the moving direction of the recording medium. CONSTITUTION:The supply opening 3 for the form is bored in the flank 2 of a housing 1 on a paper feed side and the discharge opening 5 from which a form after image formation is discharged is bored in the flank 4 on the opposite side from the flank 2 on the paper feed side. Further, a slit 7 which connects with the paper feed passage extending from the supply opening 3 to the discharge opening 5 is bored in the flank 6 perpendicular to the paper-feed side flank 2. Thus, the passage for the recording medium is formed in a slit shape, so part of the large-sized recording medium is only inserted into the passage and then an image is formed at the insertion part; and only desired pages of, for example, a notebook are inserted into the passage and then the image is formed at the insertion parts. Consequently, the image can be formed (copied) directly on the large-sized form and notebook.
