PURPOSE:To form recorded traces of prescribed intervals, by providing an area, in which recorded trace patterns of prescribed intervals and address signals whose lengths are short are formed, and another area, in which signals indicating reference rotating phase are recorded, at the inside of a recording area. CONSTITUTION:The disk D of this invention is constituted of a recording area RD which contains recorded traces of information signals by the 1st optical spot and signals recorded in lands between each recorded trace by the 2nd optical spot, 1st recorded are PGD at the inside of the recording area PGD. The area PGD has a width which is lager than the eccentricity of the disk D and fluctuation of the home position of the optical spot and a recorded trace pattern held at prescribed intervals TP and prescribed address signals Sa having lengths, at which the optical spot can pass through in a time shorter than the period corresponding to the frequency of the gain intersecting point of a tracking servo system, are previously recorded in the 1st recorded area PGD. Moreover, signals SP which indicate the reference rotating phases of the disk D are previously recorded in the 2nd recorded area SPD.
